It is an offence if you do not report a lost or destroyed MSIC to your issuing body within seven days.
To replace your lost or destroyed MSIC you must complete the below process from the relevant section of the Maritime Transport and Offshore Facilities Security Regulations (2003)
6.08R Notification of lost, stolen and destroyed blue MSICs
1) The holder of a blue MSIC commits an offence if:
a) the MSIC has been lost, stolen or destroyed; and b) the holder of the MSIC knows about the loss, theft or destruction; and c) he or she does not:
i) make a report, in the form of a statutory declaration, of the loss to the issuing body that issued the MSIC within seven days of becoming aware of the loss, theft or destruction; or
ii) if the MSIC was stolen—give the issuing body a copy of a police report, or other information issued by the police, regarding the theft, within seven days of becoming aware of the theft.
d) Penalty: 10 penalty units.
2) Strict liability applies to paragraph (1)(c).
3) However, subregulation (1) does not apply if the MSIC has been destroyed by the issuing body that issued it.
Complete a statutory declaration form relevant to your replacement circumstances, confirm that there has been no change to your circumstances since your initial MSIC application, and have it signed by a relevant authority.
Once your documents have been received, a link will be sent to your employer, for them to complete your operational need letter. This must be provided by your employer, to confirm that you still have an operational requirement to hold an MSIC.

You will need to provide some form of photo identification (Driver’s licence or passport) when lodging your request for a replacement MSIC.
This can be your Passport, Driver's Licence, Proof of Age Card etc.
The replacement cost for your lost, stolen or destroyed MSIC is $65 (GST inclusive).
Payment details will be provided via the operational need letter.
If your employer is incurring the cost of the replacement card, and is an approved Pilbara Ports account holder, then a purchase order can be provided. Pilbara Ports will invoice the company accordingly. Otherwise, credit card details can be taken over the phone.
If your employer is not incurring the cost then you will be required to pay for your replacement. Your nominated MSIC office will contact you for payment once all documents have been received. Payment can be made by EFTPOS or credit card.
Complete the below form with your current details. You will then need to upload copies of your statutory delcaration and photo ID. You can then select which office you wish for your replacement to be sent to, either Dampier or Port Hedland. This will be where you collect your MSIC from, once it has been processed.

Upon receipt of all relevant documentation a replacement MSIC can be processed and reissued to you. If you no longer reside in the local area the card can be posted by registered mail to you. If your destroyed MSIC replacement is being registered posted to you, you must return your destroyed MSIC to us before we can post your new MSIC to you.
