Maritime Transport and Offshore Facilities Security Regulations (2003) stipulates that an MSIC that is altered or defaced (permanently or temporarily) is not valid.
If your MSIC has been damaged or altered, you will need to replace your MSIC immediately.
To replace your lost or destroyed MSIC you must complete the below process from the relevant section of the Maritime Transport and Offshore Facilities Security Regulations.
6.07D Meaning of valid blue MSIC or valid temporary MSIC
1) For this division, a blue MSIC or a temporary MSIC is valid if:
a) it is issued in accordance with this division; and b) it is not expired, suspended or cancelled; and c) it is not altered or defaced (permanently or temporarily); and d) the person who shows or displays it is the person to whom it was issued.
2) A temporary MSIC is valid only in the maritime security zone or zones that, under subparagraph 6.08KA(1)(c)(v), the temporary MSIC displays or includes.
Complete a statutory declaration form outlining the details of the damage or defacing, confirm that there has been no change to your circumstances since your initial MSIC application and have it signed by a relevant authority.
Once your documents have been received, a link will be sent to your employer, for them to complete your operational need letter. This must be provided by your employer, to confirm that you still have an operational requirement to hold an MSIC.

You will need to provide some form of valid government issued photo identification when lodging your request for replacement MSIC.
This can be your Passport, Driver's Licence, Proof of Age Card etc.
The replacement cost for your Damaged or Defaced MSIC is $65 (GST inclusive).
Payment details will be provided via the operational need letter.
If your employer is incurring the cost of the replacement card, and is an approved Pilbara Ports account holder, then a purchase order can be provided. Pilbara Ports will invoice the company accordingly. Otherwise, credit card details can be taken over the phone.
If your employer is not incurring the cost then you will be required to pay for your replacement. Your nominated MSIC office will contact you for payment once all documents have been received. Payment can be made by EFTPOS or credit card.
Complete the below form with your current details. You will then need to upload copies of your statutory declaration and photo ID. You can then select which office you wish for your replacement to be sent to, either Dampier or Port Hedland. This will be where you collect your MSIC from, once it has been processed.
Your damaged or defaced MSIC card must be returned before or when collecting your replacement MSIC.

Upon receipt of all relevant documentation a replacement MSIC can be processed and reissued to you. If you no longer reside in the local area the card can be posted by registered mail to you.
