Maritime Transport and Offshore Facilities Security Regulations (2003) stipulates that a person must notify their issuing body within 30 days of their name change.
Your MSIC will cease to be valid one month after the date you changed your name.
To replace your MSIC you must complete the below process.

6.08LCA Obligation of applicants for, and holders of, MSICs—change of name
(1) A person commits an offence of strict liability if:
(a) the person is an applicant for, or a holder of, an MSIC; and
(b) the person changes his or her name; and
(c) the person does not notify the issuing body to which the application was made or which issued the MSIC (as the case requires) of the change, in accordance with subregulation (2), within 30 days after the change.
Penalty: 5 penalty units.
(2) The person must:
(a) notify the issuing body in person; and
(b) at the same time, give the issuing body an original of a current and valid document, showing the new name, which was issued to the person by a Commonwealth, State or Territory Department or agency.
Complete a statutory declaration form outlining the details of the name change, confirm that there has been no change to your circumstances since your initial MSIC application and have it signed by a relevant authority.
Once your documents have been received, a link will be sent to your employer, for them to complete your operational need letter. This must be provided by your employer, to confirm that you still have an operational requirement to hold an MSIC.

You will need to provide some form of valid government issued photo identification when lodging your request for replacement MSIC.
This can be your Passport, Driver's Licence, Proof of Age Card etc.
You will need to supply a the relevant certificate that is issued from the Department of Births, Deaths and Marriages. This could be either;
The replacement cost for your Name Change MSIC is $65 (GST inclusive).
Payment details will be provided via the operational need letter.
If your employer is incurring the cost of the replacement card, and is an approved Pilbara Ports account holder, then a purchase order can be provided. Pilbara Ports will invoice the company accordingly. Otherwise, credit card details can be taken over the phone.
If your employer is not incurring the cost then you will be required to pay for your replacement. Your nominated MSIC office will contact you for payment once all documents have been received. Payment can be made by EFTPOS or credit card.
Complete the below form with your current details. You will then need to upload copies of your statutory declaration, proof of name change and photo ID. You can then select which office you wish for your replacement to be sent to, either Dampier or Port Hedland. This will be where you collect your MSIC from, once it has been processed.
The MSIC card in your previous name must be returned before or when collecting your replacement MSIC.

Upon receipt of all relevant documentation a replacement MSIC can be processed and reissued to you. If you no longer reside in the local area the card can be posted by registered mail to you.
