Only original documents will be accepted. We will not accept photocopies, photos, scans or certified copies of any document.
If you are providing a birth certificate, it must be a full certificate (not a birth extract or commemorative certificate).
All applicants must provide identification documents from each of the following four categories:
Category A
Australian Citizenship/Naturalisation Certificate,Evidence of the start of the applicant’s identity in Australia, e.g.
- Australian Birth Certificate, or
- Australian Visa accompanied by an International Passport, or
- Australian Citizenship Certificate

Category B
A Government-issued document that provides photographic proof of the applicant’s identity and includes the applicant’s signature, e.g.
- Drivers Licence,
- Proof of Age/Photo Card, or
- Passport (Australian or foreign).

Category C
Evidence of the applicant’s use of the identity while operating in their community, e.g.
- Medicare card,
- Current ASIC or MSIC,
- Bank card, or
- PAYG summary.

Category D
Evidence of the applicant’s current residential address, if not listed on any other documents above e.g.
- Utilities bill or invoice,
- Bank statement,
- Bank account confirmation, or
- Tenancy agreement.

Change of name documents
All identification documents must contain exact name matches, unless the name variation is supported by a linking document. Acceptable examples of change of name linkage documents include;
- Marriage certificate, or
- Change of name certificate.
